Phytochemicals targeting transforming growth factor (TGF)-β and inducible nitric oxide synthase to treat rheumatoid arthritis

Rheumatoid arthritis (RA) is characterized by inflammation, progressively affects joints and causes disability. The pathogenesis of RA consists of various steps including synovial hyperplasia, inflammation, and tissue damage, which at the end results in joint destruction. Transforming growth factor beta (TGF-β) and inducible nitric oxide synthase (iNOS) are the two key targets for the treatment of RA. TGF-β is a mediator protein that has multifaceted functions, and it possesses both proinflammatory and anti-inflammatory activities depending on certain conditions. TGF-β plays its proinflammatory role by the overexpression of cytokines like interleukin-1 (IL-1) and tumor necrosis factor-alpha (TNF-α), which results in RA. TGF-β is also known as a chemoattractant for immune cells, which results in worsening inflammation in RA. While iNOS causes inflammation, it also causes tissue damage when produced in excess as in autoimmune disorders. There is an increase in levels of iNOS in the synovium of RA. Downregulation of iNOS reduces bone erosion and necrosis. We can reduce the progression of RA by targeting TGF-β and iNOS. Natural compounds targeting TGF-β and iNOS offer a potential approach to treat RA. Genkwanin, Kaempferol, Berberine, Rutin, Andrographolide, Arctigenin, 6-Shogaol, Curcumin, Chlorogenic acid, and Licochalcone A, Licochalcone C are the phytochemicals that reduce inflammation and give a potential therapeutic approach to treat RA. These phytoconstituents play the role by inhibiting NO, IL-1β, IL-6, and Prostaglandin E2 (PGE2) production, suppressing the NF-κB pathway also by influencing TNF-α and TGF-β levels and blocking JAK-2. Phytoconstituents offer promising therapeutic potential by modulating various inflammatory pathways and mediators.
Melatonin restricts periodontitis related neurodegeneration in rats

Objective: This experimental rat study aims to investigate the preventive role of melatonin in potential neurodegenerative changes associated with periodontitis in the hippocampus and cerebral cortex. Methods: Twenty-four Sprague Dawley rats were divided into three groups: Control (n = 8), Ep (n = 8), Ep + Mel (n = 8). Systemic melatonin (10 mg/kg/day) was administered to the Ep + Mel group for 35 days of periodontitis induction, starting from the application of bilateral lower first molar ligation. Following sacrification of all groups, periodontal bone loss was analyzed in Micro-CT sections (sagittal and coronal). Cerebral cortex and hippocampus tissues and serum samples were analyzed histopathologically, immunohistochemically, and biochemically. Results: Micro-CT results showed that the experimental periodontitis model was successful. When Ep + Mel and Ep were compared, a significant difference was observed in terms of histopathological damage, Caspase-3, GFAP, and Iba-1 immunopositivity (p < 0.05). Systemic melatonin administration was found to significantly reduce tissue and serum levels of IL-1β, IL-1β/IL-10, and Aβ1-42 (p < 0.05). Additionally, serum TNF-α and tissue TOS and OSI levels were significantly reduced in the melatonin group (p < 0.05). Tissue TAS and serum IL-10 showed no significant change (p > 0.05). Conclusions: This study demonstrated that systemic melatonin may limit periodontal tissue destruction and periodontitis-associated neuroinflammatory, neurooxidative, and neuroapoptotic changes in the cerebral cortex and hippocampus.

The dynamic spectrum of steatotic liver disease: the global perspective

Steatotic liver disease (SLD) comprises metabolic dysfunction-associated SLD, metabolic and alcohol-related liver disease, and alcohol-related liver disease, conditions that frequently overlap rather than fall into distinct categories. Cardiometabolic risk factors (CMRFs), including obesity, type 2 diabetes, hypertension and dyslipidaemia, are highly prevalent across SLD subtypes, with most patients exhibiting multiple metabolic abnormalities. These risks interact synergistically with alcohol exposure, accelerating fibrosis progression to cirrhosis and liver mortality. Importantly, both alcohol consumption and metabolic risks are dynamic and can fluctuate over time, leading to transitions across the SLD spectrum that static diagnostic thresholds might not adequately capture. Misclassification is common, particularly due to under-reporting of alcohol intake, underscoring the value of objective alcohol biomarkers such as phosphatidylethanol. Accurate risk stratification, therefore, requires an integrated approach combining systematic alcohol screening, structured evaluation of CMRFs and non-invasive fibrosis assessment. Management must be multidisciplinary, combining alcohol reduction strategies, optimization of metabolic control and liver-directed therapies. However, most emerging pharmacotherapies for metabolic dysfunction-associated steatohepatitis exclude individuals with concurrent alcohol use, creating a gap between clinical trial populations and real-world practice. A dynamic, spectrum-based framework incorporating repeated reassessment of alcohol and metabolic risks offers the most pragmatic path for diagnosis, treatment and equitable care delivery in SLD.
Oral–systemic health interactions in dentistry: a 25-year bibliometric and thematic mapping analysis

Aim: This study aimed to conduct a 25-year bibliometric and thematic mapping analysis to explore how the scientific literature linking oral health and systemic diseases has evolved, focusing on major trends, diagnostic developments, and emerging interdisciplinary directions. Materials and Methods: Publications indexed in the Web of Science Core Collection (2000–October 2025) under “Dentistry and Oral Medicine” were analyzed using the Biblioshiny R package and VOSviewer. Annual scientific output, citation trends, contributing countries, journals, and keyword-based thematic structures were evaluated. Results: A total of 2650 articles met the inclusion criteria. Global research output showed a steady increase, with a marked surge after 2015. The United States, China, Brazil, and England were the most productive countries, while several European nations demonstrated the highest citation impact. Periodontology journals dominated publication volume; however, recent years revealed diversification toward endodontic, peri-implant, and oral oncology topics. Thematic evolution maps indicated a transition from periodontal inflammation–oriented research themes to broader mechanistic perspectives. Conclusions: Oral–systemic health research has shifted from a periodontally centered field to a broader interdisciplinary domain integrating molecular, microbiological, and translational approaches. While significant progress has been achieved, future studies should focus on underexplored areas such as endodontic pathology, surgical interventions, and orthodontic influences to achieve a comprehensive understanding of oral contributions to systemic health.
