Kadınlarda travma sonrası stres bozukluğu: Bir olgu sunumu

Abstract

Tüm dünyada travmatik yaşam olaylarına kadınlar daha fazla maruz kalmaktadırlar. Travma sonrası stres bozukluğu (TSSB) gelişmesine yol açan etkenlerden biri de aile içi şiddettir ve Türkiye'de oldukça yaygındır. Kadına yönelik aile içi şiddet, kültürel coğrafi, dini, toplumsal ve ekonomik sınırları aşan küresel bir sorundur. Travmanın ciddiyeti, süresi, eş zamanlı farklı travmatik olayların varlığı, kişilik yapısı, travma öncesinde var olabilen psikiyatrik hastalık öyküsü TSSB gelişmesi olasılığını arttırır. Bu çalışma, bir olgu sunumudur. 42 yaşındaki hastamızda, TSSB belirtileri yanında, bunlara eşlik eden depresif belirtiler de mevcuttu. Bu çalışmada, TSSB tanısı düşünülen kadınlarda TSSB’ye eş tanılı durumlar, TSSB için kolaylaştırıcı faktörler ve tedavi yöntemlerinin tartışılması amaçlanmıştır. Psikiyatrik hastalık öyküsü, psikodinamik ve psikososyal öykü ve psikolojik testler temelinde psikodinamik formülasyon yapılmıştır.

Women are more exposed to traumatic life events in all over the world. Prevalence of domestic violence is very high in Turkey and domestic violence is one of the underlying reasons of Posttraumatic Stress Disorder (PTSD). Domestic violence against women is a common problem experienced by all women of the world in all societies and various social groups even its prevalence, intensity and type changes. Severity of trauma, its duration, presence of comorbid different traumatic events, personality traits and history of psychiatric disorders increases the likelihood of development of posttraumatic stress disorder (PTSD). This study is a case report. 42 years old patient had symptoms of PTSD as well as depressive symptoms that accompanied them since puberty. This study discussed the diagnosis of PTSD in women, comorbid conditions to PTSD, facilitating factors for PTSD, and treatment methods. Psychodynamic formulation has been reached on the basis of evaluation of the psychiatric disorder history, psychodynamic and psychosocial history and psychological tests.
